If your submission is incomplete, your application will not be accepted.
The 2012 Scholarship Dinner will be held on June 15, 2012.
IMPORTANT: Application Submission Procedure
You must submit all of the items listed below to complete your
application packet. There are two methods to do this:
A) By mail - Submit FIVE (5) completed application packets to the
address on Page 1 postmarked by March 30th, 2012. Each of the five
application packets must have one copy of each item below in order to
be complete.
B) Electronically - Use a scanner to scan each of the items below to
your computer. (If a scanner is not available, you may complete
the application on-line and then mail required documents to our address at
the top of Page 1 postmarked by March 30th, 2012.) Click on the Online
Application link above. Fill in the online version of the application. When
you reach the last page, you can use the "Select Files" buttons to
select and upload each of the required documents.
Regardless of the method of application submission, a complete packet
consists of:
1. Completed application (Pages 1 through 4) including the three essays.
2. Official High School Transcript.
3. Complete 2011 tax returns for your family and yourself (if you
file). Please include all pages of your tax returns as well as your
W-2 forms. If your family does not file US tax returns, please email
us for further instructions.
4. FAFSA
5. Two recommendations from two separate individuals (prefer at least
one recommendation from a teacher).
Again, if you application is incomplete, your application will NOT be accepted.
